To register with Woodlands Surgery you need to fill out a registration form with your details e.g. Name, Date of Birth, Your Address, Telephone Number and details of the previous Doctor you were registered with. You are invited to make an appointment with one of our Practice Nurses for a New Patient Check should you wish to do so. The receptionist registering you will ask if you have a preference to be seen by a particular doctor. You do not have to state any preference and are always able to make an appointment to see any of the doctors working at the practice. If you want to change your preference at any time you may do so by informing us either in person or in writing.
If you have a medical card, please complete the change of address section, not forgetting postcode and telephone number and bring it along to reception.

If you are unable to find your medical card, you will need to collect a registration form (GMS1) from reception or you can download a copy by clicking on the link below. Please fill this form in carefully, remembering to include all postcodes, telephone numbers and date of birth.

If you are only staying in our area for a short period of time (less than 3 months), then we will ask you to fill in a Temporary resident form (GMS3) which you can download by clicking on the link below. You will of course be treated by the Doctor in the usual way.

You have the right to express a preference to receive services from a particular clinician and we will record this preference. We will endeavour to do this, but please note this cannot be guaranteed.

• Know the opening times
• Find out how to arrange home visits, repeat prescriptions & telephone consultations
• You may not always need to see a doctor; find out what services the nurses provide
• Keep your appointment or cancel it
• Take a list of questions with you when you see the doctor or nurse
• Tell the receptionist if you think you need more than 10 minutes; she may be able to help
• Think twice before you make an appointment; have you tried simple home treatments or asked your pharmacist?
